Traumatic osteochondral lesions of the talus.

چکیده
The ankle is a common site of sports injuries. Extreme violent inversion of the ankle associated with sports accidents, subjects it to traumas of varying degrees. Fractures of the medial and lateral malleoli, and tears of the ligaments are easily recognisable. When these are excluded on the X-ray examination, the diagnosis of ankle sprain is usually made and fast spontaneous recovery is expected. However, some of these patients continue to suffer from pain, limitation of movement and long periods of sport incapacity. These cases of unresolving "ankle sprain" were related to post traumatic osteochondral lesions of the talus (Smith et al, 1977).

UNLABELLED Osteochondral (OCD) lesions of the talus are frequent sequelae of traumatic ankle injuries such as ankle sprains, fractures, and recurrent ankle instability. Initial management of talus lesions in most cases involves arthroscopy and microfracture/curettage. Tissue resulting from the microfracture is fibrocartilage. Clinical improvement in pain is seen in approximately 75% to 85% of p...

Osteochondral lesions of the talus that have previously undergone surgical management or involve the shoulder region of the talus require methods other than microfracture for treatment. For those lesions that have failed microfracture and do not involve the shoulder of the talus, osteochondral autologous transfer is our preferred treatment of choice. Perpendicular access to the lesion, often by...

Osteochondral lesions of the talus (OLT) are rare joint disorders. The talus is the third most common location of this disorder, following the knee and elbow joints. OLT represents 4% of all osteochondral lesions in the body. This article discusses the surgical treatment and postoperative rehabilitation of osteochondral lesions of the talus.

Osteochondral lesions of the talus can present as a late complication of ankle injuries. As the talus is largely covered by articular cartilage, it has a limited ability for repair. Early and accurate diagnosis is important as talar integrity is required for optimal function of the ankle. The common presentation is chronic ankle pain with a history of ankle trauma. Conservative treatment involv...
